How Do We Sign Up?

It is easy to sign your school up for the 2012 Foodbank WA School Breakfast Program! There are 2 sections necessary for your application:

1. Complete the 2012 Foodbank WA School Breakfast Program Registration form

2. Attach a Letter of Support

Foodbank WA School Breakfast Program Registration Form

The registration form outlines the terms of the Foodbank WA School Breakfast Program. Please complete all information requested on pages 2 and 5 and return these pages to Foodbank WA.

The Registration form must be completed in full – incomplete forms will be returned for completion. Download the 2012 Registration form here »

Letter of Support

This is a brief letter outlining the demonstrated need within the school community that has prompted you to establish / continue a school breakfast program. Each school is assessed on its own merit, so it is imperative to include this letter.

Submission

Please send the following to the the Foodbank WA Perth office:

• Completed Pages 2 and 5 of the registration form

• the Letter of Support

Please retain a copy of your application.

Please allow up to a week for processing. Upon approval, the principal & coordinator will be advised (via email) of successful registration & supplied with the appropriate order form.
